The SNV HUB provides support and access to services for everyday needs. This program is free to Clark County residents.
Your Community. Your Health. Our Support.
The Southern Nevada Pathways Community HUB (SNV HUB) connects you with community health workers (CHWs) who can help you with many things related to your health and well-being. Think of them as guides to a healthier life. Here are some of the things CHWs can help you with:
Your CHW will meet with you to talk about what you and your family need. You will create a plan and work together for as long as you need assistance. This program is completely free for you and your family.
To qualify, you must be 18 or older, live in Clark County, Nevada, and meet one of the following conditions:
- Have prediabetes
- Have been diagnosed with gestational diabetes
- Have type 2 diabetes
- Be pregnant

- Certified Support: Our CHWs are trained and certified to provide the best care and support.
- Community Focus: We work with many local partners to make sure you get the help you need, no matter where you are in Clark County.
What languages are available?
We provide services in both English and Spanish to best support our community.
How much does it cost?
All SNV HUB services are entirely free of charge. This means there’s absolutely no cost to you or your family.
