The SNV HUB provides support and access to services for everyday needs. This program is free to Clark County residents.

Your Community. Your Health. Our Support.

The Southern Nevada Pathways Community HUB (SNV HUB) connects you with community health workers (CHWs) who can help you with many things related to your health and well-being. Think of them as guides to a healthier life. Here are some of the things CHWs can help you with:

Life’s Challenges

Finding jobs, access to healthy food, housing and transportation and more

Health Basics

Connecting to doctors and dental care providers, exploring health insurance options, addressing maternal health needs, offering medication management resources and much more

Mind, Emotions, and Well-Being

Addressing mental health challenges and providing support to manage stress and difficult emotions providing resources to address substance misuse and empowering you to create healthy habits

Your CHW will meet with you to talk about what you and your family need. You will create a plan and work together for as long as you need assistance. This program is completely free for you and your family.

To qualify, you must be 18 or older, live in Clark County, Nevada, and meet one of the following conditions:

  • Have prediabetes
  • Have been diagnosed with gestational diabetes
  • Have type 2 diabetes
  • Be pregnant

    We help you find and get the services you need, like health care, housing, and food.

    Our trained CHWs work with you to understand your needs and help you every step of the way.

    We keep track of how you’re doing to make sure you get the best help possible.

The SNV HUB serves residents of Clark County, Nevada, who are 18+ years of age and who meet one of the following conditions:
