SilverSummit Pilot: Improving Maternal and Infant Health Outcomes
The SNV HUB implemented a one-year maternal health pilot with support from a 2023 SilverSummit Community Reinvestment Grant. The pilot was done in partnership with Dignity Health, St. Rose Dominican, who employed three community health workers to screen and serve pregnant women referred to the SNV HUB.
The purpose of the pilot was to demonstrate the effectiveness of the Pathways Community HUB Institute® Model for high needs SilverSummit Medicaid members.
Key pilot outcomes:
- 92 pregnant women participated in the pilot
- Of SilverSummit members enrolled, 80% were from the health plan’s “hard to reach” list
- 91% of participants had pregnancy outcome improvements
