Comagine Health Wraps Successful 3-Year Nevada ACL Project

The Nevada three-year Administration for Community Living (ACL) cooperative agreement with Comagine Health concluded in April 2025. This award gave Comagine Health the opportunity to establish the Wellness Integrated Network of Nevada (WINN) – Nevada’s first collaborative network dedicated to building the infrastructure needed to deliver and sustain chronic disease self-management education (CDSME) programs across the state.

ACL project key achievements:

• Enrollment of 716 Nevadans into workshops, with 413 completing a CDSME or support program – exceeding project enrollment and completion targets
• Programs for Walk With Ease and Camine Con Gusto were implemented in Nevada for the first time
• Partners in Nevada now have free access to Compass for promoting their workshops and managing their data
• The WINN Business and Strategic Plan was completed and will guide the coalition into the next phase

WINN will continue to grow through the Advancing Health Equity to Address Diabetes model and in partnership with the Nevada Department of Health and Human Services.
