About the Pathways Community Advisory Council

The Pathways Community Advisory Council (PCAC) consists of individuals who live and work in Clark County and who guide the development of the Southern Nevada Pathways Community HUB (SNV HUB). The PCAC supports community engagement and participation efforts, provides direction for the participant and community health worker (CHW) experience, reviews data, and brings input from community partners to continually improve implementation of the SNV HUB.

The PCAC works closely with the SNV HUB staff to identify and address issues in the community through analysis of the SNV HUB data (e.g., gaps, resources, etc.) and plays a pivotal role in quality improvement of community-based care coordination and alignment of community resources.

The PCAC is committed to improving the health of the community. In partnership with the PCAC, the SNV HUB regularly communicates and reports its strategies, progress and challenges to its funders, policymakers, care coordination agency (CCA) members, participants and the community at large.

The PCAC provides counsel to the SNV HUB in the following key areas:

PCAC Functions and Responsibilities

The PCAC provides counsel to the SNV HUB in the following key areas:

Strategic Leadership, Direction, and Guidance

  • Advise on the guiding principles, equity approach, and community engagement strategies with input from clients, staff, committees, and community
  • Guide and monitor key priorities
  • Provide accountability for initiatives
  • Advise on partner agency engagement

Health Care Equity and Community Needs

  • Share experiences and knowledge to understand, address, and respond to community needs, particularly those unserved or underserved
  • Assist with aligning available community resources with identified participant needs
  • Advise how the SNV HUB can help address health care equity for hard-to-reach community members

Data-Driven Decision-Making

  • Use data collected by the SNV HUB network to inform decision-making and the direction of implementation, growth, and quality improvement strategies
  • Participate in SNV HUB data analysis to identify areas of improvement and address community-identified issues and resource gaps

Service Delivery and Improvement

  • Ensure the SNV HUB is effective, efficient, and beneficial to the Southern Nevadan community members with the highest health and social needs
  • Play a key role in improving care coordination and community resource alignment
  • Assist with service delivery coordination and quality improvement areas of focus
  • Identify activities and strategies to support the effectiveness and sustainability of community-based organization (CBO) services
  • Evaluate and report on how participants are using services and the ability of Community-Based Organizations (CBOs) to meet that demand

Community-Based Organization Network Development

  • Identify core competencies for CCAs and establish objective standards for CCA credentialing and network certification
  • Engage potential CBO network members and CCAs for the purpose of understanding existing barriers to further development of the network
  • Advise and evaluate feedback to proposed standards

Deliverable Achievement

  • Monitor priorities and provide accountability for initiatives
  • Interact with the SNV HUB staff regarding strategy, community engagement and shared measurement
  • Assist in identifying and securing funding and sustainability elements

Community Engagement and SNV HUB Championship

  • Identify alignments between member’s organization and network with the SNV HUB’s mission, purpose and goals
  • Advocate for the SNV HUB and support of the CHW workforce
  • Identify opportunities to present and share the work to raise awareness and elicit consideration and feedback from a broad range of key partners
  • Serve as a vocal champion of the SNV HUB’s collective impact in the community
  • Support network growth through identification of additional partnerships with CCAs, referral partners, and payers
  • Openly share resources, best practices, measurement tools and success stories with the SNV HUB, other organizations and community members

PCAC Membership

The PCAC includes diverse members from multidisciplinary fields who have lived experience in health care and/or social services, who offer a diverse range of perspectives and backgrounds, who have subject matter expertise in the 21 Standard Pathways, and who are committed to improving health and social outcomes for Southern Nevadans.

Membership in the PCAC is composed of community leaders and partners including local residents, past participants, care coordination agencies (CCAs) and CHW staff, referral partners, and direct service providers. The PCAC strives for diverse representation from the health care sector, including:

  • City, County, and State Government
  • Community-Based Organizations
  • Faith-Based Organizations
  • Community Health Centers
  • Foundations
  • Hospitals and Clinics
  • Health Plans
  • Medicaid Agencies
