Contracting with the Southern Nevada Pathways Community HUB (SNV HUB) connects health plans and members to a proven and established model of care coordination that successfully serves hard-to-reach individuals and improves health and social outcomes.

Hospitals, community organizations, health plans, local governments and beyond are looking at how to successfully address the social determinants of health and provide holistic wrap-around care to under-resourced community members. It cannot be done in silos or by duplicating efforts. Collaboration across local community organizations, with health care systems and social service sectors, improves the overall health of patients when they collectively use the Pathways Community HUB Institute® Model (PCHI® Model) and collectively work toward solutions that provide care and resources to residents and improve outcomes.

SNV HUB Infrastructure

Our network of local organizations is central to the SNV HUB infrastructure as it provides care coordination services and addresses individually modifiable risk factors for disproportionally affected residents of Southern Nevada. These risk factors include:

Studies by Medicaid managed care plans that contract with certified Pathways Community HUBs (PCHs) have demonstrated a return on investment (ROI) and measured health improvement for members who received PCHI Model community-based care coordination.

The PCHI Model exemplifies three key elements by leveraging the skills of community health workers in its approach to serving individuals who have been economically/socially marginalized.

236% ROI

For every dollar spent on the Pathways Community HUB intervention for its members, there was a savings of $2.36. Additionally, for new babies born to mothers with complex health care needs enrolled in a PCH, there was a per member per month cost savings of $403 during the first year of life compared to those born to mothers not enrolled at delivery.

Why Contract with the SNV HUB?

Expand Care Coverage

Contracting with the SNV HUB enables health plans with access to a scalable network of trained care coordination agencies that are driven by local community health workers (CHWs). The CHWs are trained to engage members and address modifiable risk factors for hard-to-reach members. This model is structured to optimize the role and scalability of the CHW workforce, supplementing the capacity of care teams by freeing up clinicians to work at the top of their licenses.

Enhance Provider Participation and Simplify Administration

The SNV HUB provides health plans with readily accessible to a scalable, community-embedded care coordination workforce and simplifies the contracting process by managing a standardized network of care coordination agencies who are trained and supported in the evidence-based PCHI Model. The SNV HUB manages all agency subcontracts, training, quality improvement, program fidelity and network expansion to meet demand.

Improve Health Outcomes for Members

The SNV HUB is well positioned to be integrated as part of a comprehensive strategy for addressing Nevada Medicaid and commercial plan priorities. To date, Comagine Health’s SNV HUB has demonstrated results in improving maternal and infant health outcomes, supplementing workforce shortages and addressing health-related social needs.

Support Ongoing Evaluation and Innovation

The SNV HUB has established systems for providing plans with ongoing data reports and dashboards to support real-time monitoring of member enrollment, progress and outcomes.
