National Diabetes Prevention Program
The National Diabetes Prevention Program (National DPP) lifestyle change program was developed specifically for people who have prediabetes or are at risk of developing type 2 diabetes, but who do not already have diabetes. The program can help prevent or delay the onset of diabetes by learning and practicing healthy lifestyle skills.

This lifestyle change program is not a fad diet or an exercise class. And it’s not a quick fix. It’s a year-long program focused on long-term changes and lasting results. A year might sound like a long commitment, but learning new habits, gaining new skills and building confidence takes time. Program participants:
- Get support from a trained coach and a close group of their peers
- Learn to manage stress and overcome challenges
- Discover fun physical activities to do alone or with a group
- Learn how to make healthy food choices they can enjoy
- Make small lifestyle changes over time
As you begin to eat better and become more active, you’ll notice changes in how you feel and maybe even in how you look.
Research shows that CDC-recognized lifestyle change program participants who lost 5-7% of their body weight and added 150 minutes of exercise per week cut their risk of developing type 2 diabetes by up to 58% (71% for people over 60 years old).
Even a decade later, program participants were one-third less likely to develop type 2 diabetes than individuals who did not join a program. The program can also lower your risk of having a heart attack or stroke, improve your health, help you feel more energetic and even reverse your prediabetes diagnosis.

Key components of the program include:
- CDC-approved curriculum with lessons, handouts and other resources to help you make healthy changes.
- A lifestyle coach, specially trained to lead the program, to help you learn new skills, encourage you to set and meet goals, and keep you motivated. The coach will also facilitate discussions and help make the program fun and engaging.
- A support group of people with similar goals and challenges. Together, you can share ideas, celebrate successes and work to overcome obstacles. In some programs, the participants stay in touch with each other during the week. It may be easier to make changes when you’re working as a group than doing it on your own.
All CDC-recognized lifestyle change programs follow a CDC-approved curriculum and discuss the same topics over the year. Your lifestyle coach will adapt the sessions to match your group’s background, interests and needs.
For instance, your lifestyle coach may:
- Show you how to prepare healthy versions of popular local or ethnic foods and ask for ideas from group members
- Provide tips for eating healthy during cultural holidays or events
- Share fun local events that can help you be physically active
- Give you handouts to address your specific concerns
There’s also a Spanish-language curriculum, so some programs are offered completely in Spanish.
The program runs for one year.
- During the first six months of the program, you will meet about once a week.
- During the second six months, you’ll meet once or twice a month.
You may think you learned enough in the first six months and can skip the second half of the program, but you’d be cheating yourself!
Making lifestyle changes is an ongoing process. Staying in the program for the full year is essential to help you stick to new habits and avoid slipping back into old habits. And if you have not reached your goals in the first half of the program, your lifestyle coach and other group members can help you succeed.
In the second half of the program, you will enhance the skills you’ve learned so you can maintain the changes you’ve made. These sessions will review key ideas such as tracking your food and physical activity, setting goals, staying motivated and overcoming barriers. The lifestyle coach and small group will continue to support you. You may learn some new information, too.

Diabetes Self-Management Education and Support
Diabetes Self-Management Education and Support (DSMES) provides an evidence-based foundation to help people with diabetes navigate their condition. This includes daily self-management decisions to manage blood sugar, improve overall health and prevent or delay diabetes complications. People who participate in DSMES have been shown to have better diabetes-related outcomes than those who do not.

DSMES is support that helps YOU manage your diabetes. There are two types of programs available:
Diabetes Self-Management Program (DSMP) is a six-week class for people living with type 2 diabetes. DSMP teaches techniques to deal with the symptoms of diabetes, such as fatigue, pain, stress, and emotional challenges such as depression, anger, fear and frustration.
Class topics include:
- Healthy eating
- Exercise at your level
- How to use glucose monitoring to make day-to-day decisions
- How to manage sick days
- Appropriate use of medication
- Reducing risk for other health conditions
- Coping with the unique challenges of living with diabetes
The Diabetes Conversation Map program is a unique and engaging experience for people living with diabetes and was developed in collaboration with the American Diabetes Association.
Taking place over eight weeks, Conversation Map is led by a certified facilitator who uses 3-by-5-foot “maps” to engage the class in discussion and goal setting. By being personally involved in the learning process, you are able to discover solutions on your own and use your newfound knowledge to make positive lifestyle changes.
Enhance®Fitness
EnhanceFitness is a proven senior fitness and arthritis management program that improves endurance, strength, balance and flexibility. It reduces arthritis symptoms with safe, low-impact exercises led by certified instructors in a relaxed atmosphere that promotes fun, laughter, friendship and smiles. It helps older adults at all levels of fitness become more active, energized and empowered to sustain independent lives. Of all the program’s multiple benefits, the one that matters the most is that it’s a great time!

Fit & Strong®
Fit & Strong! is a group physical activity and behavior change program for people with arthritis. The program consists of 24 sessions, each lasting 90 minutes.
- The first 60 minutes focus on flexibility, low-impact aerobics and strength training to improve lower body function.
- The remaining 30 minutes consist of health education and group problem-solving using a structured curriculum.
Participants develop personalized action plans to maintain physical activity and progress after the class ends. Fit & Strong! is effective, engaging and fun!

Healthy Heart Ambassador Blood Pressure Self-Monitoring
Healthy Heart Ambassador Blood Pressure Self-Monitoring (HHA-BPSM) is a program that features personalized support for developing a blood pressure self-monitoring routine, tips for maintaining cardiovascular health and nutrition education. The simple process of checking and recording blood pressure at least twice a month over a period of four months has been shown to lower blood pressure in people with hypertension.
Working with a Healthy Heart Ambassador, individuals with a diagnosis of high blood pressure and no recent cardiac complications learn how to regulate their blood pressure through a combination of home self-monitoring of blood pressure, individualized support and nutrition education for better blood pressure management.
Participants agree to measure and record their blood pressure at least two times per month, attend two personalized consultations per month, and attend monthly nutrition education seminars.
Joining the HHA-BPSM program is an investment in your health and a commitment to reducing the risk for heart attack or stroke.
Healthy Steps for Older Adults
Healthy Steps for Older Adults is for seniors who are interested in improving their fall prevention skills and need a place to begin. It is a two-day, two-hour class that covers safety topics like proper footwear, safety at home, good nutrition for bone health and the importance of exercise for adults age 65 and older.

Stepping On
Stepping On is a seven-week program designed for people who are living at home and have experienced a fall or are concerned about falling.
The program is an evidence-based program that is effective and proven to reduce falls in older people. The program incorporates strategies to implement positive lifestyle changes to keep participants independent, upright and active.
